Terms and Conditions of Service

Upon your instruction of any representative of Social Work Reports Ltd to undertake a service, you have committed to these terms and conditions.

  1. Relationship

The person who engages and or instructs Social Work Reports Ltd shall be considered to be our client.

 2. Risk

The person who engaged us shall provide us with any details of any physical or psychological risk their client has demonstrated to others

3. Fees

We charge £40 per hour for travelling time from our offices in Newark, Nottinghamshire and 0.45ppm. If travelling for more than four hours in one direction we will charge for overnight accommodation as an expense. Without exception  funds from local authorities and solicitors must be received within 14 days of receiving funds from LAA etc. All privately commissioned assessments must make 100% payment prior to the assessment being confirmed. All reports will be paid for in full even if the report is not used.

4. Property

All reports prepared by Social Work Reports Ltd remain the property of Social Work Reports Ltd until payment is received in full, including any late payments charged. Reports will not be released until full payment is received. 

 5. Interpreters

If the assessed person or persons cannot speak English interpreters shall be provided at the expense of the person requesting the assessment.

6. Confidentiality and limitations

All interviews and subsequent records made are kept in the strictest of confidence in accordance with the Data Protection Act.

We will not disclose any details in connection with your client except where there is immediate and serious risk to the health or safety of a third party or your client.

7. General Data Protection Regulations

In accepting your instructions, I agree to be data controller jointly with you of the personal data you send me. You agree to respond to any subject access requests or other requests made by data subjects in respect of this information on my behalf as well as your own. If I receive a subject access or other request regarding information you have sent me, I will send it to you to deal with, along with any other personal data I hold as a result of accepting your instructions.

8. Late Cancellations

Please be sure when you make the appointment that you can definitely attend.

We require 48 hours’ notice if you cannot attend your appointment with us. If you cancel within 48 hours of your appointment, we will charge you for the full amount.

Cancellations made with more than 48 hours’ notice can be rescheduled with no penalty. 

9. Refunds

In the event that refunds are agreed, they will be made minus the cost of any work already commenced.
